The first session of the series will provide an overview of the various payment vehicles that states use to better reimburse providers for treating Medicaid and indigent patients—and the regulatory framework of those payment programs. For example, Medicaid spent $263 billion on hospital care in 2022, and roughly 40% of that total spend was through supplemental payments. Congress took action to curb this spending through the One Big Beautiful Bill Act, but supplemental payments remain a vital (and even more-regulated) component of the healthcare safety net.

Because Medicaid is a state and federal partnership, every Medicaid dollar must have some form of nonfederal share provided by the state or a local government entity. This session will also address common ways states finance these payments, including provider taxes and intergovernmental transfers. Although Congress and the Centers for Medicare & Medicaid Services (CMS) allow various payment options, they also mandate strict compliance with an extensive regulatory system to ensure responsible management of federal expenditures and safeguard patient access. Nearly every provider group in the healthcare-industrial complex participates in some form of Medicaid supplemental payments, and this presentation will provide an overview and insights into the future direction of these programs.
